Does anyone try folding the dice designed by Tanaka Masashi?

After my third try (that didn't rip), I have something that resembles a cube (needs to be adjusted to be fastened more securely.) However, after my 2nd and 3rd tries, I still wonder how to make the face with the 5 on it. I can manage to make 4 in the correct positions but for the 5th dot, the corner is too thick or something. For my 5th face, I seem to have a leftover flap attached to the 4th face so I wonder if that's right? On this 3rd try, I did manage to make 5 spots but I created the 5th spot from that flap and it doesn't seem right...

Could anyone help?

Edit: Actually, I need help on the 6 face too...

Now THAT is genius. The kind of thing you bring to a party to amaze your friends. :D I'm going to try to fold it sometime soon...
