Where can I find this box??

I was wondering where I can find this box? I'm assuming it's a fuse but does anyone know which book it is in???

Hey, I was looking at that box and thought it absolutely gorgeous! I am familiar with the base hexagonal box, but would love to make that top. Unfortunately, when I clicked on the link, the server told me that the page no longer existed. I was heart broken! Does anyone know where I could find a diagram, CP, or something of the sort? Or would anyone be willing to take a picture of it unfolded (a very quick sort of CP), so that I could make it? Thanks a bunch!
