I'm pretty sure that this is a model by Sok Song. The diagrams might be in his book "Crease+Fold."

I checked. It's not in Crease+Fold neither in Creased magazines

It seems to be a model of Stefan Delecat, from Germany.
The pdf diagram is under the title "$-Hemd mit Krawatte" on the linked page.
(Perhaps it is not exactly the same model, because on your picture, there are creases on the right and left of the tie going to the arms...)
