this is a modified Satoshi Kamyia wizard, done by Phillip West. I'm pretty sure there are no diagrams for it. but maybe somebody made a CP? but I highly doubt it.

you may be able to achieve it by folding the base of the wizard and shaping it to this, it doesn't look that much different from the actual model.
