First of all, I want to know whether it is from one paper
if so, then the author of the diagram/cp, which book has it

The author is Jon Tucker, diagrams and CP aren't available to my knowledge. The paper is painted American foil.

Hi, I am the author. It is indeed folded from one uncut square of American foil paper. A crease pattern or diagram isn't available now, but may be published in the future :)
